Excellent article, Mish - it's about time this was highlighted - it is happening all the time here in the UK, with insurance companies, 'phone companies, etc.
I found that I had entered into a 'continuous payment authority' or CPA, when I believed it was a one-off payment (for insurance). But further amounts of money were removed from my account. The only way I could stop it was to contact the bank, who cancelled the card and issued a new one. Moral: read all the t's and c's first, before paying any entity using a card.
